Yes, we are deeply committed to working with Children but did you know all of our therapists are skilled with delivering services to Adolescents, Adults, Families & Couples​. Please visit Rayze for clients over 15 years of age.

TYPES OF REFERRALS:

▪ Self-Referrals

▪ Referrals from Doctors/Paediatrician's with or without a Medicare referrals

▪ National Disability Insurance Scheme (NDIS) - Plan & Self Managed

▪ Third Party Referrals - whereby another person or agency will be paying for the service.

Please note - Supersaurus does not provide services (including reports) for Family Law or other Legal matters.
We recommend you find an alternate provider for these matters.

WHO NEEDS TO COMPLETE THIS FORM?

▪ All new clients are required to complete our online form.

▪ Existing clients that have not participated in an appointment with any of our therapists for more than 12 months

▪ Clients seeking an Assessment

TRIAGE

New Clients will be scheduled into a 15 - 20 minute

telephone triage appointment upon receipt of their New Client Form.

This is a COMPLIMENTARY service.

Telephone triage is supported by evidence-based guidelines, experience, training and knowledge, they would then:

▪ Interpret symptoms and answers to carefully structured questions to determine the severity of the condition

▪ Recommend clients to the appropriate level of health care therapy/health professional

• This is not a guarantee that we can offer the services required for you/your child.

NDIS Clients - the Initial Service Agreement is discussed and completed based on the information gathered in this telephone triage appointment. It is then emailed to the Representative or the Client for electronic signing. If you have difficulty with online signing, please advise Triage and we can arrange for it to be printed and signed at Reception. Appointments will not be scheduled until the Service Agreement is signed.
